How it Works
- It starts with connecting you to a primary care provider. He or she will direct a team of Total Health Care specialists who are all working as one team for you.
- Together, your team will write a custom care plan for you to help you achieve your best health.
- Your team will provide continuous, coordinated care throughout your lifetime to ensure you get the best care possible, no matter what you need.
- Your team will also coordinate all the services you need, when you need them—from doctors’ appointments and tests to medication reviews and social services.
Because it’s “patient-centered,” you are always in charge of your care. Your health care team is there to educate you and help you make the best decisions for you and your family.
